Rules of the Contest
- Enter as many times times as you would like.
- Optional Short Story for any and all entries is a plus.
- Must be 18 years old to enter.
- Each entry must include at least one recognizable
Couture Cupcake in video or photograph.
- Each entry must be Football related.
- Each entry must include a digital copy
of video or photograph.
- Each entry must include our release form
(see below at www.SpringsCupcakes.com)
allowing Couture Cupcakes to use video or
photograph for advertising, marketing, etc.
- All entries must be the property of the person entering,
cannot use videos or photos taken or owned by others.
- Entries can be submitted via email to CoutureCupcakeCo@aol.com or you can drop off at
the Cinema Store at 3347 Cinema Point, Colorado
Springs, CO 80922 during regular business hours.
- All digital videos and photographs become the
property of Couture Cupcakes and will not
be returned under any circumstances.
- Direct ALL questions via email to
CoutureCupcakeCo@aol.com.
No Phone calls please.
- All entries must be received by 6:00 pm Mountain
time on January 29, 2012 to be considered.
- No late entries allowed.
- Winners will be contacted and posted on
Facebook by 6pm February 1, 2012.

I, the undersigned, do hereby consent and agree that ,
its employees, or agents have the right to take photographs,
videotape, or digital recordings of me beginning on and
ending on and to use these in any and all media, now or hereafter known, and exclusively for the purpose of
advertising, marketing, store artwork, etc.

I do hereby release to Couture Cupcakes, its agents,
and employees all rights to exhibit this work in print
and electronic form publicly or privately and to
market company.
I waive any rights, claims, or interest I may have to
control the use of my identity or likeness in whatever
media used.

I understand that there will be no financial or other
remuneration for recording me, either for initial or
subsequent transmission or playback.

I also understand that Couture Cupcakes is not
responsible for any expense or liability incurred as a
result of my participation in this recording, including
medical expenses due to any sickness or injury
incurred as a result.

I represent that I am at least 18 years of age,
have read and understand the foregoing statement,
and am competent to execute this agreement.

All media when submitted become the property of
Couture Cupcakes, and will not be returned
under any circumstances.
